Bath Rog and Uric Acid: K Khane, K Nakhane? A Nepali Food List

If your uric acid is high or you have gout, you can usually eat all kinds of daal, green vegetables, low-fat milk and dahi, and eggs; eat rice and roti in measured portions. Do not eat organ meats such as liver and kidney, or sukuti; cut back on meat; and limit alcohol (especially beer) and sugary soft drinks. Many people have heard that "once you have uric acid, you must stop eating daal", but studies show that plant foods such as daal do not noticeably raise the risk of gout. The list below is Dr. Krishna Adhikari's advice. The general diet rules are in our gout and uric acid article.

This list is Dr. Krishna Adhikari's advice for patients with high uric acid or gout. If you have kidney disease, heart disease or diabetes, or your own doctor has given you separate advice on food, ask your doctor before you change what you eat.
Fine to eat (खान हुने)
Daal and beans: red lentils, mung, pigeon pea, black gram, chickpeas, peas, kidney beans, horse gram, soybean (मुसुरो, मुग, रहर, मास, चना, केराउ, राजमा, गहत, भटमास)
Vegetables: leafy greens, spinach, mustard greens, cauliflower, cabbage, mushrooms (साग, पालुङ्गो, रायो, काउली, बन्दा, च्याउ)
Grains: rice, roti and dhido in measured portions; where possible millet (kodo) or buckwheat (phapar) dhido, or whole-wheat roti (भात, रोटी, ढिँडो; कोदो/फापरको ढिँडो, चोकरसहितको आटाको रोटी)
Milk and dahi: low-fat milk, plain dahi, buttermilk (कम बोसोको दूध, सादा दही, मोही)
Eggs (अण्डा)
Fruit: fresh fruit, in normal amounts (ताजा फलफूल, सामान्य मात्रामा)
Drinks: water (पानी)
Eat less (कम खाने)
Meat: goat, mutton, buff, pork and chicken (खसी/बोका, भेडा, राँगा/बफ, सुँगुर, कुखुराको मासु)
Fish: fresh fish (ताजा माछा)
Other dairy: paneer, ghee, cream; sweetened dahi, lassi, juju dhau (पनिर, घिउ, क्रिम; चिनी हालेको दही, लस्सी, जुजु धौ)
Alcohol, especially beer: rakshi (spirits), beer, jaand, chhyang, tongba (रक्सी, बियर, जाँड, छ्याङ, तोङ्बा)
Sugary drinks: soft drinks such as Coke and Fanta, energy drinks (चिसो पेय, एनर्जी ड्रिङ्क)
Tea: very sweet tea (धेरै चिनी हालेको चिया)
Avoid (नखाने)
Organ meat: liver, kidney, tripe, brain (कलेजो, मृगौला, भुँडी, गिदी); spleen (फियो)

Why?
Daal, spinach, cauliflower and mushrooms contain purines, but studies show that plant foods like these have little effect on the risk of a gout attack. In a study of more than 51,000 people in Singapore, those who ate more soybean and other legumes had a lower risk of gout.
Organ meats and meat are high in purines, and the body turns purines into uric acid. In measurements from Japan, chicken breast had more purine (141 mg per 100 g) than mutton (96 mg), which is why chicken is also on the "eat less" list.
Drying removes water, so dried fish and meat are high in purines. Sidra is a dried fish. The US health institute NIAMS advises avoiding small fish such as sardines and anchovies, and prawns and other shellfish.
Beer raises the risk of gout a lot, and rakshi (spirits) raises it too. We found no separate study on jaand, chhyang or tongba; treat them like beer.
Drinks with sugar or fructose raise the risk of gout.
You do not need to give up chicken and fresh fish, but keep portions moderate. In the Singapore study, people who got more of their protein from chicken and fish had a slightly higher risk of gout.
What to eat and not eat with high uric acid (uric acid huda k khane k nakhane): common questions
Is curd (dahi) good for uric acid?
Dr. Krishna Adhikari's advice: yes, plain low-fat dahi. Dahi and milk contain almost no purine. In a large US national survey, people who ate yogurt at least every other day had slightly lower uric acid than people who ate none. The Arthritis Foundation and the National Kidney Foundation also recommend low-fat milk and dahi. But this evidence comes mostly from observational studies, so the American College of Rheumatology's treatment guideline makes no specific recommendation on dairy.
Sweetened dahi, lassi and juju dhau contain sugar, so have them less often. Paneer is also low in purine, but it is high in fat, so eat it in small amounts.
Can I eat eggs?
Dr. Krishna Adhikari's advice: yes. Hen's eggs contain almost no purine. An analysis combining five US studies found slightly lower uric acid in people who ate eggs, and a large Singapore study found no link between eggs and gout. So eggs, daal and dairy are protein choices in place of meat.
What should I drink?
Water: drink plenty through the day. Water helps flush uric acid out in the urine. If your doctor has told you to limit fluids because of heart or kidney disease, follow that advice.
Milk and buttermilk (mohi): low-fat milk is fine. So is mohi.
Tea (chiya): tea has not been shown to raise or lower uric acid, but a lot of sugar in it has sugar's effect. Use less sugar.
Coffee: regular coffee drinkers have been seen to have lower uric acid, but this is only an observational finding. If you do not drink coffee, there is no need to start for this reason.
Lemon water: lemon water without sugar is fine to drink. For the claim that it "removes" uric acid, see the home remedies section below.
Alcohol: drink less rakshi, beer, jaand, chhyang and tongba, especially beer.
Sugary drinks: drink fewer soft drinks, such as Coke and Fanta, and energy drinks.
Do not drink: fruit juice, packaged or fresh-squeezed.

What may help
Water: drink plenty; being short of water can bring on a gout attack.
Weight: if you are overweight, losing weight with a lower-calorie diet and exercise can lower uric acid and reduce gout attacks. But avoid crash diets.
Rakshi and beer: people who cut down on alcohol or stop have been found to have lower uric acid.
Sugary drinks: drink fewer soft drinks, and give up juice.
What can bring on an attack: eating a lot of meat and drinking a lot of alcohol at once, as at Dashain and Tihar feasts, can bring on a gout attack.
Claims seen in the news: what the evidence says
Lemon water: a small study that looked back at the records of 90 patients at two clinics in Poland found lower uric acid after 6 weeks of drinking the juice of two lemons in 2 litres of water every day. It was not a randomized controlled trial, and the 2 litres of water may itself have made a difference. Lemon water without sugar is fine to drink, but we found no evidence that it can replace medicine.
Garlic: we found no trial in people testing garlic for gout or uric acid. In one trial, people with high blood pressure who were on blood-pressure medicine took an aged black-garlic extract for 12 weeks, and their uric acid fell slightly (0.3 mg/dL); but that trial was about blood pressure, not uric acid, and it did not test garlic in food. None of the other papers we found tested whether garlic lowers uric acid in people; they were lab, animal or computer studies, reviews or a survey. Garlic is fine in cooking, but do not rely on it as treatment.
Ajwain (carom seeds): the only study we found was in rats, and it used a mix of ajwain, black pepper and one other plant. Its authors wrote that no clear conclusion can be drawn.
Vitamin C tablets: some studies show vitamin C tablets lower uric acid slightly, but in a trial in patients with gout the effect was too small to matter for treatment. The American College of Rheumatology recommends against adding vitamin C tablets in gout. Ask your doctor before taking them.
Please note: keep taking any uric acid-lowering medicine your doctor has prescribed, even when the symptoms have gone; do not stop it on your own because of home remedies.
Bath rog: what to eat and what to avoid?
Bath rog is not one disease. Gout, rheumatoid arthritis, osteoarthritis and many other diseases are all called bath in everyday speech, and each is treated differently. The list above is for uric acid and gout. For rheumatoid arthritis or osteoarthritis, ask your doctor about diet.
If you have osteoarthritis and are overweight, losing weight can help reduce pain and make the joints easier to move, especially weight-bearing joints such as the knees and hips.

बाथ रोगमा के खाने के नखाने?
